General Motors has decided that certain 2027 Chevrolet Bolt EV, 2025-2026 Cadillac OPTIQ, Chevrolet Colorado, Equinox EV, GMC Canyon, 2026 Buick Enclave, Envision, Cadillac CT5, Escalade, Escalade ESV, Escalade IQ, Escalade IQL, LYRIQ, VISTIQ, Chevrolet Blazer EV, Corvette Convertible, Corvette Coupe, Corvette Convertible E-Ray, Equinox, Silverado 1500, 2500, 3500, Silverado EV, Suburban 1500, Tahoe, Traverse, GMC Acadia, Hummer EV Pickup, Hummer EV SUV, Sierra 1500, 2500, 3500, Sierra EV, Terrain, Yukon, and Yukon XL vehicles. The radio may not have been set to the correct status to download the electronic owner's manual during production. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, “Occupant Protection.”

Risk: Without an owner's manual to consult, owners may not know how to safely use and operate the vehicle, increasing the risk of injury in a crash.

Fix: Dealers will reset the vehicle radio, which will facilitate automatic download of the electronic owner’s manual, free of charge. Owner notification letters are expected to be mailed April 13, 2026. Owners may contact GM customer service at 1-866-467-9700, Chevrolet customer service at 1-800-222-1020 or Cadillac customer service at 1-800-333-4223. Forward Collision Avoidance3 issues
criticalFORWARD COLLISION AVOIDANCE
Filed: Jan 2026

Your forward collision avoidance system's sensor is failing, causing a 'Service Driver Assist System' warning light to appear every time you start the car. The automatic braking feature has engaged unexpectedly while you were slowing for a red light, causing sudden hard braking that resulted in whiplash. The sensor under the front bumper corroded due to exposure to dirt and mud on back roads, even though it was supposed to be weatherproof. The repair will involve replacing the faulty sensor unit under the front bumper.

Your car's forward collision avoidance system and automatic emergency braking activated while you were driving, and the warning light came on. The dealer looked at it but didn't diagnose or fix the problem. The system may be malfunctioning or detecting obstacles that aren't actually there. A technician will need to scan the vehicle's computer and inspect the forward collision sensors and camera for faults or calibration issues.

Filed: Jan 2026

While driving at about 45 mph, your SUV's forward collision avoidance system suddenly activated the emergency brakes without warning, startling you, your passengers, and the driver behind you. You immediately turned off the emergency braking system. The system may need to be checked to determine why it engaged when there was no actual collision risk.

Service Brakes,forward Collision Avoidance2 issues
criticalSERVICE BRAKES,FORWARD COLLISION AVOIDANCE
Filed: Feb 2026

Your brake system suddenly lost all braking performance without any warning, making the vehicle unsafe to drive. A Chevrolet dealership confirmed the master brake cylinder and related brake components are defective. The vehicle has been out of service since January 2026 waiting for replacement parts that are currently on backorder. Fixing this will require replacing the faulty master brake cylinder and potentially other brake system components.

criticalSERVICE BRAKES,FORWARD COLLISION AVOIDANCE
Filed: Feb 2026

Your forward collision avoidance system is malfunctioning and causing dangerous brake problems. The system appears to brake suddenly without reason—especially in rain and at night—and sometimes refuses to stop completely, forcing you to release and reapply the brakes with extra force. You're also getting false alerts from side-view sensors claiming vehicles are approaching when nothing is there, which is particularly concerning since this nearly caused a rear-end collision at a traffic light. The system may be misinterpreting oncoming traffic or road conditions as obstacles in your path.

NHTSA #: 11715624
Steering1 issue
criticalSTEERING
Filed: Mar 2026

Your power steering has stopped working, making it difficult to steer your vehicle. This is a serious issue that affects your ability to control the car safely. The repair will likely involve inspecting and servicing the power steering system, which may include replacing the power steering pump, hoses, or fluid.

NHTSA #: 11728164
Power Train,electrical System,fuel/propulsion System1 issue
criticalPOWER TRAIN,ELECTRICAL SYSTEM,FUEL/PROPULSION SYSTEM
Filed: Mar 2026

Your 2026 Chevrolet Equinox EV failed to start at 1,260 miles, displaying multiple warning messages about battery faults and charging problems. The front wheels locked and wouldn't roll, preventing you from shifting to neutral, which required a tow truck to move the vehicle. The dealership performed several resets and reprogramming but never identified or fixed the underlying electrical problem, and when the same issue returned a week later with rapid battery drain, it was closed again without a permanent repair.

criticalPOWER TRAIN,ELECTRICAL SYSTEM,SERVICE BRAKES
Filed: Mar 2026

While driving your 2026 Chevrolet Equinox EV home from the dealership, the "Service Park Assist System" warning came on and stayed on continuously. During the same drive, the automatic emergency braking system suddenly activated and applied the brakes hard even though there were no objects behind or around your vehicle—creating a serious risk of being rear-ended by following traffic. The dealership found that the left rear radar sensor lost communication with the vehicle's computer network (diagnostic code U1126), and replaced the sensor module after a two-week wait for parts. The repair will likely involve diagnosing and fixing the vehicle's electrical communication network to prevent similar radar and braking system failures.

Electrical System,unknown Or Other,engine1 issue
criticalELECTRICAL SYSTEM,UNKNOWN OR OTHER,ENGINE
Filed: Mar 2026

Your car unexpectedly shifted from Drive to Park while traveling at 65 mph on the way home from the dealership, forcing your husband to manually shift back to Drive. The vehicle then wouldn't start and displayed all warning lights, though it eventually restarted with a persistent check engine light. The next day, the car shifted into Park on its own five more times during the drive back to the dealership, and once there, the dealership couldn't shut the engine off. This shifting problem is a serious safety issue that needs immediate electrical and transmission diagnostic work.

Air Bags1 issue
criticalAIR BAGS
CrashFiled: Feb 2026

While driving your rented 2026 Chevrolet Equinox at 35 mph, a deer ran into the vehicle's path and you collided with it. The passenger-side front end sustained severe damage, but the airbags did not deploy during the crash. You were injured with lower abdomen pain, though you didn't seek medical care. The reason why the airbags failed to deploy has not been determined.

Service Brakes1 issue
criticalSERVICE BRAKES
Filed: Feb 2026

Your car's brake master cylinder has failed prematurely according to Chevrolet service. This is the component that pressurizes brake fluid to make your brakes work. The repair will involve replacing the brake master cylinder with a new one.

NHTSA #: 11717192
Forward Collision Avoidance,lane Departure1 issue
criticalFORWARD COLLISION AVOIDANCE,LANE DEPARTURE
Filed: Feb 2026

Your Forward Collision Avoidance and blind spot detection systems are giving false warnings — alerting you to vehicles or objects that aren't actually there, especially when it's raining. In a more serious incident, your brakes failed to stop the vehicle when you pressed the pedal at 30 mph, and the car accelerated on its own instead, nearly causing a rear-end collision. The fix will likely require your dealer to diagnose and recalibrate these safety systems, possibly including software updates or sensor replacements.

NHTSA #: 11715605
Power Train1 issue
criticalPOWER TRAIN
Filed: Jan 2026

Your 2026 Equinox has a gear shift lever on the steering wheel that looks and functions like a windshield wiper lever. When it rains or you instinctively reach to activate the wipers, you may accidentally pull the gear shift and put the car into neutral while driving, which has happened to other drivers on highways and major roads. Getting the car back into drive isn't intuitive, and losing power while driving at speed creates a serious safety hazard. A redesign or warning system is needed to prevent accidental gear shifts.

NHTSA #: 11709597
Seat Belts1 issue
criticalSEAT BELTS
Filed: Jan 2026

Your driver's side seat belt isn't locking or restraining properly while driving. When you pull on the shoulder belt across your chest, it fails to lock and you can stretch it all the way to the steering wheel. The lap belt also won't lock securely and can be pulled upward excessively. This is a safety concern since the seat belt won't hold you in place during an accident. The fix will likely involve inspecting the seat belt retractor mechanism and replacing any faulty locking components.

NHTSA #: 11709230
Electrical System,lane Departure1 issue
highELECTRICAL SYSTEM,LANE DEPARTURE
Filed: Mar 2026

Your blind spot monitoring system frequently gives false alerts when there's actually no vehicle nearby, especially in rain or wet conditions. Because these warnings happen so often without reason, you might start ignoring them, which means you could miss a real warning when you actually need it during a lane change. This creates a safety risk since the system is supposed to help protect you, but instead trains you to distrust it.

NHTSA #: 11725067
Seat Belts,forward Collision Avoidance,lane Departure1 issue
highSEAT BELTS,FORWARD COLLISION AVOIDANCE,LANE DEPARTURE
Filed: Feb 2026

Your Equinox is experiencing multiple electrical and safety system issues: the blind spot warning indicator isn't working properly, your seat sensors trigger false alerts even when unoccupied, the cruise control cuts out unexpectedly, the vehicle brakes on its own without reason, and the information display screen goes black randomly. These problems involve interconnected safety and convenience systems that will likely require comprehensive electrical diagnostics and possible software updates or sensor replacements.

NHTSA #: 11715792
Engine1 issue
highENGINE
Filed: Jan 2026

Your brand new Equinox started leaking oil from the front end just four weeks after purchase. Despite four dealership visits and even a complete engine replacement, the oil leak persists. The dealer has advised you to keep driving it while they work on another repair attempt, though you're concerned about safety.

NHTSA #: 11711655
Unknown Or Other1 issue
mediumUNKNOWN OR OTHER
Filed: Mar 2026

Your Equinox EV produces an unusual high-pitched "angelic" sound when in Drive or Reverse at low speeds or while stopped, which plays both inside and outside the vehicle up to about 22 mph. The sound stops abruptly once you exceed the speed threshold, which may be distracting to you as the driver. The owner believes this sound doesn't effectively warn pedestrians or children that a vehicle is present, and suggests the vehicle should be modified to use a more natural warning sound, reduce the exterior volume, and allow drivers to disable the interior speaker.

NHTSA #: 11727806
Visibility1 issue
mediumVISIBILITY
Filed: Jan 2026

Your windshield has a visible distortion that makes it harder to see clearly while driving. When you took it to the dealer, they suggested the distortion might be related to your sunglasses rather than a defect in the glass itself, so no repair was performed. The distortion was noticeable around 2,000 miles of driving. Getting the windshield replaced or repaired would involve removing the old glass and installing a new one.
