NHTSA Campaign: 16V096000

Component: SEAT BELTS:REAR/OTHER

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2006-2012 RAV4 vehicles manufactured July 28, 2005, to December 19, 2012, and 2012-2014 RAV4 EV vehicles manufactured July 24, 2012, to August 29, 2014. In the event of a frontal collision, the rear seat belts for the outboard seats may be cut by the seat cushion frame.

Risk: A cut seat belt may not properly restrain the seat occupant, increasing their risk of injury in a crash.

Fix: Toyota will notify owners, and dealers will install a protector over both rear seat cushion frames, free of charge. The recall began on June 24, 2016. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is G0F.

NHTSA Campaign: 14V054000

Component: SERVICE BRAKES, HYDRAULIC:ANTILOCK/TRACTION CONTROL/ELECTRONIC LIMITED SLIP:CONTROL UNIT/MODULE

Toyota is recalling certain model year 2012 and 2013 Toyota Tacoma and Lexus RX350 vehicles and certain model year 2012 Toyota Rav4 vehicles. In the affected vehicles, the brake system contains a brake actuator that adjusts the fluid pressure of each wheel cylinder. An electrical component within the actuator may experience an increased resistance resulting in the illumination of various warning lights, including those for the Vehicle Stability Control (VSC) system, Antilock Brake System (ABS), and Traction Control system.

Risk: If this occurs, these systems could become inoperative, reducing the directional control assistance, increasing the risk of a crash.

Fix: Toyota will notify owners, and Toyota and Lexus dealers will update the software for the Skid Control electronic control unit, free of charge. The recall began on March 28, 2014. Owners may contact Toyota at 1-800-331-4331.

NHTSA Campaign: 15V577000

Component: VISIBILITY/WIPER

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2009-2012 RAV4 vehicles manufactured October 27, 2008, to December 19, 2012 and 2012-2014 RAV4 EV vehicles manufactured July 24, 2012, to August 29, 2014. In the affected vehicles, water may drip onto the joint that connects the wiper linkage to the wiper motor.

Risk: Over time, the dripping water may cause corrosion and wear at the joint, potentially resulting in the separation of the wiper linkage. Once this occurs, the wipers would not function, reducing driver visibility and increasing the risk of a crash.

Fix: Toyota will notify owners, and dealers will replace the water diverter channel. In addition, a new wiper linkage with a wiper motor crank arm will be installed. The wiper linkage will have a protective cover over the wiper linkage joint to minimize water splash and a retainer ring to prevent the wiper arm from separating from the wiper motor. The recall began on November 7, 2016. Owners may contact Toyota's customer service number at 1-800-331-4331.

NHTSA Campaign: 13V014000

Component: AIR BAGS:FRONTAL:SENSOR/CONTROL MODULE-INACTIVE

Southeast Toyota Distributors, LLC (SET) is recalling certain models interspersed through model years 2009 through 2013 as follows: model year 2009-2012 Tacoma, 4Runner, Camry, Camry Hybrid, Prius, and RAV4; model year 2009-2010 Avalon, FJ Cruiser, and Highlander Hybrid; model year 2010-2013 model year Corolla, Sienna and Tundra; model year 2009-2013 Highlander and Venza; model year 2012 Prius V; and model year 2010-2012 Sequoia. During modification by SET to include accessories such as leather seat covers, seat heaters or headrest DVD systems, these vehicles may not have had the passenger seat occupant sensing system calibration tested. Without passing the calibration test, the occupant sensing system may not operate as designed.

Risk: If the front passenger seat occupant sensing system is out of calibration, the front passenger airbags may not deploy or they may deploy inappropriately for the passenger's size and position. This could increase the risk of personal injury during the event of a vehicle crash necessitating airbag deployment.

Fix: Southeast Toyota will notify owners, and dealers will test the sensitivity of the occupant detection sensors, and recalibrate them as necessary. The recall began on March 21, 2013. Owners may contact Southeast Toyota at 1-800-301-6859.

NHTSA Campaign: 13V123000

Component: EQUIPMENT:OTHER:LABELS

Southeast Toyota is recalling certain model year 2008 and 2010-2013 Toyota Tundra, 2010-2012 Rav4, 2012 Toyota Sequoia, 2010-2011 Toyota Corolla, 2010-2011 Toyota Camry and Camry Hybrid, 2010-2013 Toyota Highlander and Highlander Hybrid, 2010-2013 Toyota FJ Cruiser, 2011 Toyota Land Cruiser, 2010-2013 Toyota Venza, 2010-2011 Toyota 4Runner, 2010-2013 Toyota Tacoma, 2011-2012 Toyota Sienna, 2012 Toyota Prius, 2013 Scion FR-S, 2011 Scion XD, 2011 Scion XB, and 2012 Scion TC vehicles. These vehicles were sold with labels that were outside the allowable one percent of accuracy of actual weight added. Thus, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) Number 110, "Tire Selection and Rims."

Risk: An inaccurate label could lead to owners overloading their vehicles and tires. An overloaded vehicle can result in a tire failure which may result in a vehicle crash, personal injury, or property damage.

Fix: Southeast Toyota will notify owners and provide a corrected label with instructions concerning its installation. A small group of the affected vehicles will need additonal remedies which are still being developed. The recall began on May 28, 2013. Owners may contact Southeast Toyota at 1-800-301-6859.

Your engine, 4WD, and traction control warning lights came on while driving, and your truck felt like it was slipping gears before suddenly shutting down and smoking near a railroad track. The top radiator hose coupling broke, spilling antifreeze everywhere. After replacing the hose and refilling the antifreeze and water, your truck won't start—it just sputters when you try to crank it. The repair may involve checking the engine for damage caused by overheating, replacing spark plugs, and ensuring the cooling system is properly bled of air.

Your engine maintenance light came on, and then your RAV4 unexpectedly accelerated on its own multiple times while driving home, lurching forward toward other vehicles even when you weren't pressing the gas pedal. You pressed the brake firmly and put the car in neutral, but the engine continued revving to 4,000–5,000 RPM. This happened four times during your drive and created serious safety risks, though it hasn't happened since and mechanics haven't been able to reproduce the problem. The issue likely involves the fuel system's throttle control or electronic engine management system and may require diagnostic scanning or replacement of accelerator components.

Your windshield wipers are moving slowly, changing direction unexpectedly, and stopping in the middle of the windshield when you turn them on. This problem happened before and was supposedly fixed under a Toyota recall, but it's back again. The wipers need to be inspected and likely replaced or the wiper motor and linkage system needs to be repaired.

criticalSTEERING,POWER TRAIN,SERVICE BRAKES
Filed: May 2025

Your car unexpectedly accelerated to 60 mph on its own while in neutral at the end of a car wash and wouldn't respond to steering or braking for over half a mile, causing severe damage to the left front wheel. While no crash occurred, this represents a serious loss of vehicle control. The issue may involve the throttle system, brake system, or steering components and will require professional diagnostic testing to identify the root cause.

criticalVEHICLE SPEED CONTROL
Filed: Mar 2025

Your engine has revved and accelerated on its own multiple times over several years while you were braking to stop, with the most recent incident occurring in March 2025 when the car lurched in reverse and then accelerated even harder in park. You've confirmed your foot was only on the brake, the floor mat was secure and in the correct position, and nothing was touching the gas pedal. This issue stops when you shift to neutral, suggesting a problem with the engine control system or throttle mechanism rather than driver input.

Filed: Aug 2024

Your car's airbags deployed suddenly while driving at 65 MPH without any impact or warning, injuring both the driver and passenger. The driver suffered burns to the neck and skeletal injuries, while the passenger had skeletal injuries, cuts, and throat issues requiring medical attention. The vehicle was towed and the manufacturer opened a case, but the dealer was unable to diagnose or repair the issue at the time.

Electrical System1 issue
criticalELECTRICAL SYSTEM
CrashFiled: May 2024

Your car has experienced multiple instances of losing electrical power while turning or braking, causing the engine to stall. This happened again on January 18th, 2024, and then once more on January 22nd while braking—this time resulting in a rear-end collision because you lost power and steering control. Your mechanic has replaced battery connectors multiple times, but the problem persists; you believe a known battery issue affecting 2013–2018 RAV4s may also apply to your 2012 model. Resolving this will likely require battery inspection and possible replacement, along with checking all electrical connections.

Service Brakes1 issue
criticalSERVICE BRAKES
Filed: Sep 2023

When you press the brake pedal, your engine revs up and the car moves forward instead of stopping. This is a serious safety issue that needs immediate attention. The problem likely involves the brake system or engine control components working together incorrectly.

NHTSA #: 11546369
Fuel/propulsion System1 issue
criticalFUEL/PROPULSION SYSTEM
Filed: Jun 2022

Your car suddenly lost all power to the wheels without any warning lights, messages, or sounds—it happened while stopped at a red light and had to be pushed to safety by others. A Toyota service specialist found that the splines (the connecting ridges) on the rotor had sheared off, meaning the entire engine needs to be replaced. This repair will involve removing and replacing your engine with a new or rebuilt unit.

Engine And Engine Cooling,engine1 issue
highENGINE AND ENGINE COOLING,ENGINE
Filed: Feb 2026

Your 2012 RAV4 is shuddering while driving at various speeds and burning through engine oil faster than normal. Multiple warning lights are coming on, and diagnostic scans found electrical fault codes (P0607, P1603, P1604) that an independent mechanic couldn't fully identify. Fixing this will likely require electrical system diagnosis and possibly engine oil system repairs.

Filed: Nov 2025

Your radiator coolant hose connector adapter has a small pipe that can become brittle and break off, causing coolant to leak out of your vehicle. This deterioration happens over time and can leave you without coolant, potentially causing engine overheating. The fix would involve replacing the upper radiator hose assembly or the connector adapter with a new one.

Your car's windows fog up constantly regardless of which climate control setting you use—defroster, fan, fresh air, or AC. The problem gets worse when you use the front defroster in bad weather, and rolling down the windows is the only temporary workaround, creating a serious safety hazard since you can't see clearly while driving. This issue likely stems from excessive moisture buildup inside the vehicle cabin that the ventilation system isn't removing properly. A mechanic will need to diagnose whether the problem is caused by a failed HVAC blend door, clogged cabin air filter, or an issue with the heating and air conditioning system itself.

Your car has multiple issues: the brakes require constant service and attention, the sun visors won't stay in place and keep breaking, and your engine is consuming a lot of oil without any visible leaks. You may notice the oil on the dipstick looks black and burnt shortly after you add fresh oil or get an oil change. These issues likely require brake system inspection, sun visor replacement, and engine diagnostics to identify why it's burning through oil so quickly.

Filed: Oct 2024

Your transmission may need replacement due to a faulty torque converter, even though your vehicle has had regular maintenance and hasn't been in an accident. You've discovered that Toyota issued service bulletins addressing this issue for some RAV4 owners, but your 2012 model wasn't included in the recall, despite many other owners experiencing the same problem. A new torque converter or transmission replacement will likely be needed to restore proper operation.

Filed: Oct 2023

Your check engine light, traction control light, and 4WD warning light are coming on while you drive. This problem has been happening on and off for about two months. You've scheduled a service appointment with the dealer, but the vehicle hasn't been diagnosed or repaired yet. The issue started around 125,000 miles. Diagnosing this will likely involve the dealer scanning your engine computer for fault codes and checking the traction control and 4WD systems.

Filed: Jul 2023

After getting a wheel alignment at the dealership on 6-6-23, your RAV4 started wobbling on the freeway just three days later. The dealership diagnosed worn rear tow arms with excessive play and movement, which they said the alignment service should have caught. They quoted $1,000 to replace both rear tow arms. A replacement of the rear tow arms would involve removing the old ones and installing new suspension components.
