Dec 2016

NHTSA Campaign: 16V596000

Component: SUSPENSION:REAR

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2006-2011 Toyota RAV4 vehicles manufactured October 31, 2005, to September 7, 2010, and 2010 Lexus HS250h vehicles manufactured July 6, 2009, to August 26, 2010. The affected vehicles have rear suspension arms (rear tie rods) with adjusting lock nuts that may have been improperly tightened after servicing. As a result of being loose, the arm may have thread damage and may rust, possibly leading to the failure of the arm and an abrupt change in the vehicle's alignment.

Risk: Failure of the rear tie rod could cause a loss of vehicle control, increasing the risk of a crash.

Fix: Toyota will notify owners and dealers will replace both rear suspension arms and encapsulate the locknuts, free of charge. The recall began on September 30, 2016. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are G0V for Toyota vehicles and GLK for Lexus vehicles. Note: This recall supersedes recall 13V-383, in which some vehicles were only inspected, sealed, and did not have both arms replaced. The affected vehicles were previously recalled under 13V-383, but did not have both rear suspension arms replaced at that time. Those vehicles need to have both arms replaced as part of this recall remedy.

NHTSA Campaign: 15V577000

Component: VISIBILITY/WIPER

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2009-2012 RAV4 vehicles manufactured October 27, 2008, to December 19, 2012 and 2012-2014 RAV4 EV vehicles manufactured July 24, 2012, to August 29, 2014. In the affected vehicles, water may drip onto the joint that connects the wiper linkage to the wiper motor.

Risk: Over time, the dripping water may cause corrosion and wear at the joint, potentially resulting in the separation of the wiper linkage. Once this occurs, the wipers would not function, reducing driver visibility and increasing the risk of a crash.

Fix: Toyota will notify owners, and dealers will replace the water diverter channel. In addition, a new wiper linkage with a wiper motor crank arm will be installed. The wiper linkage will have a protective cover over the wiper linkage joint to minimize water splash and a retainer ring to prevent the wiper arm from separating from the wiper motor. The recall began on November 7, 2016. Owners may contact Toyota's customer service number at 1-800-331-4331.

Component: VISIBILITY:POWER WINDOW DEVICES AND CONTROLS

Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.

Risk: If the sliding electrical contacts are not protected by lubricant, debris and moisture that get into the switch may cause a short circuit and the switch assembly may overheat and melt, increasing the risk of a fire.

Fix: Toyota will notify owners, and dealers will inspect the switch, applying a lubricant if no abnormality is found. If abnormality is found, the PWMS circuit board will be replaced, free of charge. The recall is expected to begin December 20, 2015. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is C0M.

NHTSA Campaign: 13V014000

Component: AIR BAGS:FRONTAL:SENSOR/CONTROL MODULE-INACTIVE

Southeast Toyota Distributors, LLC (SET) is recalling certain models interspersed through model years 2009 through 2013 as follows: model year 2009-2012 Tacoma, 4Runner, Camry, Camry Hybrid, Prius, and RAV4; model year 2009-2010 Avalon, FJ Cruiser, and Highlander Hybrid; model year 2010-2013 model year Corolla, Sienna and Tundra; model year 2009-2013 Highlander and Venza; model year 2012 Prius V; and model year 2010-2012 Sequoia. During modification by SET to include accessories such as leather seat covers, seat heaters or headrest DVD systems, these vehicles may not have had the passenger seat occupant sensing system calibration tested. Component: EQUIPMENT:OTHER:LABELS

Southeast Toyota is recalling certain model year 2008 and 2010-2013 Toyota Tundra, 2010-2012 Rav4, 2012 Toyota Sequoia, 2010-2011 Toyota Corolla, 2010-2011 Toyota Camry and Camry Hybrid, 2010-2013 Toyota Highlander and Highlander Hybrid, 2010-2013 Toyota FJ Cruiser, 2011 Toyota Land Cruiser, 2010-2013 Toyota Venza, 2010-2011 Toyota 4Runner, 2010-2013 Toyota Tacoma, 2011-2012 Toyota Sienna, 2012 Toyota Prius, 2013 Scion FR-S, 2011 Scion XD, 2011 Scion XB, and 2012 Scion TC vehicles. These vehicles were sold with labels that were outside the allowable one percent of accuracy of actual weight added. Thus, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) Number 110, "Tire Selection and Rims."

Risk: An inaccurate label could lead to owners overloading their vehicles and tires. An overloaded vehicle can result in a tire failure which may result in a vehicle crash, personal injury, or property damage.

Fix: Southeast Toyota will notify owners and provide a corrected label with instructions concerning its installation. A small group of the affected vehicles will need additonal remedies which are still being developed. The recall began on May 28, 2013. Owners may contact Southeast Toyota at 1-800-301-6859.

Owner-Reported Issues (20 of 284)

Air Bags2 issues
criticalAIR BAGS
Filed: Mar 2024

Your car's side curtain airbags deployed unexpectedly while a mechanic was working inside the vehicle, even though there was no crash. The mechanic was kneeling on the driver's seat reinstalling the center console after replacing the emergency brake cable when the airbags suddenly inflated. This unexpected deployment is concerning because it suggests the airbag system may be oversensitive or malfunctioning. The repair will likely require replacement of the deployed side curtain airbags and a diagnostic inspection of the airbag control module and sensors to determine why they activated without impact.

NHTSA #: 11574877

CrashFiled: Oct 2023

Your car's side curtain airbag failed to deploy during a side-impact crash, resulting in serious injuries to a passenger. This is a critical safety system failure that should be investigated immediately to determine why the airbag did not protect the occupant as designed.

NHTSA #: 11552442
Steering2 issues
highSTEERING
Filed: Oct 2025

Your steering wheel is very stiff and doesn't return to center after turning, making the car difficult to drive. This may be related to a manufacturing defect in the electronic control unit (ECU) that operates the electric power steering system. You should have a Toyota dealer diagnose and repair the steering control module.

NHTSA #: 11691140
highSTEERING
Filed: Aug 2023

In hot weather, your steering wheel becomes stiff and difficult to turn, and the car doesn't straighten out properly after you complete a turn. This makes the vehicle harder to control in hot conditions. The issue likely involves the power steering system, which may need fluid replacement or inspection of the steering components.

FireFiled: Sep 2024

Your 2011 RAV4 caught fire in your driveway and was destroyed. While there's a known recall for fires affecting 2013 RAV4s, your 2011 model experienced the same problem, suggesting the issue may affect other model years beyond what's currently covered by recall.

NHTSA #: 11615978
Engine,fuel/propulsion System1 issue
criticalENGINE,FUEL/PROPULSION SYSTEM
Filed: Jun 2024

Your car's accelerator pedal continued to speed up even after you released it, forcing you to brake hard and shift to Park to stop the engine. The car operated normally after restarting, but this has happened multiple times and feels unsafe. You've ruled out floor mat interference and suspect the accelerator pedal itself is sticking intermittently. A mechanic should inspect the pedal assembly and throttle system for mechanical sticking or binding.

NHTSA #: 11596966
Engine,vehicle Speed Control1 issue
criticalENGINE,VEHICLE SPEED CONTROL
Filed: Jan 2024

Your car unexpectedly accelerated and rolled forward while you were stopped at a red light with your foot on the brake. This happened again when you shifted from Park to Drive. An independent mechanic found that the canister purge valve and throttle body needed attention, and the vehicle was repaired. The repair likely involved replacing the purge valve and cleaning or servicing the throttle body to restore proper engine idle and acceleration control.

NHTSA #: 11567309

criticalSTEERING,SERVICE BRAKES,FUEL/PROPULSION SYSTEM
CrashFiled: Sep 2023

While parking at a shopping center, your brakes suddenly locked up and became unresponsive, while simultaneously your steering wheel locked in place. When you released the brake pedal, the car suddenly accelerated forward uncontrollably despite your attempts to brake again, causing you to roll into a metal obstruction. There were no warning lights or sounds before or during this failure. The repair will likely involve a complete diagnostic inspection of the brake system, steering system, and engine control systems to identify what caused these three systems to fail simultaneously.

Vehicle Speed Control,service Brakes1 issue
criticalVEHICLE SPEED CONTROL,SERVICE BRAKES
CrashFiled: Sep 2023

While slowly backing into a parking space at about 5 mph with your foot on the brake, your car suddenly accelerated with a loud engine rev instead of stopping, crashing into a concrete wall. The airbag deployed from the impact and caused a fracture in your hand. You're concerned the brake sensor system may not be working properly, preventing the brakes from engaging when you pressed the pedal.

NHTSA #: 11542024
Service Brakes,forward Collision Avoidance1 issue
criticalSERVICE BRAKES,FORWARD COLLISION AVOIDANCE
Filed: Aug 2023

When you press the brake pedal firmly while parking, your car suddenly accelerates instead of slowing down, with the engine loudly revving above 1000 RPM and lurching the vehicle forward 1-10 mph. This has happened 4 times over three months and is happening more frequently, creating a hazard in tight parking spaces near walls or pillars. The issue appears to be a malfunction in either the brake system or the forward collision avoidance system interfering with normal braking. A technician will need to diagnose why the brakes and engine are behaving unexpectedly when you're trying to stop.

NHTSA #: 11538848
Vehicle Speed Control1 issue
highVEHICLE SPEED CONTROL
Filed: Feb 2026

Your RAV4 unexpectedly accelerates even when you press the brake pedal, but only when cruise control is turned on (even if not actively set). The vehicle stops normally when you shift to neutral or press harder on the brake. This happened while driving and while backing out of your driveway. A mechanic indicated Toyota has a known fix for this issue, though your vehicle hasn't been diagnosed or repaired yet.

NHTSA #: 11720240
Electrical System1 issue
highELECTRICAL SYSTEM
Filed: Feb 2026

You discovered that your 2011 Toyota RAV4 had incorrect mileage on the odometer when you purchased it. The odometer showed 146,000 miles at purchase, but state records revealed the actual mileage was 190,000 miles. This is odometer fraud, which typically involves replacing or tampering with the instrument cluster.

highELECTRICAL SYSTEM,VEHICLE SPEED CONTROL
Filed: Jun 2025

While reversing to park your car, it unexpectedly accelerated on its own. You pressed the brake and the vehicle stopped normally. After you turned off the engine and went inside, the car restarted itself without you starting it. No warning lights appeared on the dashboard. The issue occurred around 80,000 miles. This could involve inspecting the electronic throttle control system, brake sensors, and the engine control computer to prevent unintended acceleration or unwanted restarts.

NHTSA #: 11669248
Tires1 issue
highTIRES
Filed: Mar 2025

You brought your car to a tire shop for a rotation and provided a new Yokohama tire to replace a worn front tire. The shop installed your tire, but when you drove away, it made a loud noise. When you returned, the shop had moved the tire to the rear and replaced it with a different, worn-out Dimax tire that has a different speed rating than your other tires. The shop refused to return your original Yokohama tire. Mismatched tire speed ratings can affect handling and safety, especially in wet conditions like rain.

NHTSA #: 11651391
Engine1 issue
highENGINE
Filed: Dec 2024

Your engine is consuming oil much faster than normal — you're losing about 4 quarts between oil changes and only 1,000 miles of driving. The check engine light didn't come on until the engine actually stopped, and now you have to check the oil level every day instead of just at regular intervals. This has been happening for almost 4 years. The fix will likely involve diagnosing the source of the leak or excessive burning (such as worn piston rings, valve seals, or a gasket leak) and repairing or replacing the damaged components.
