NHTSA Campaign: 12V537000
Component: STEERING
Toyota is recalling certain model year 2004-2009 Prius vehicles manufactured August 5, 2003, through March 30, 2009. The affected vehicles have a steering intermediate extension shaft assembly built with metal splines that complete the connection from the steering wheel to the steering gear. These splines may have been manufactured with an insufficient hardness.
Risk: Over time, the splines may wear and eventually fail resulting in a loss of steering ability, increasing the risk of a vehicle crash.
Fix: Toyota will notify owners, and dealers will inspect the extension shaft and replace it if it is one with insufficient hardness, free of charge. The recall began during December 2012. Owners may contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331.
NHTSA Campaign: 12V536000
Component: ENGINE AND ENGINE COOLING
Toyota is recalling certain model year 2004-2009 Prius vehicles manufactured August 6, 2003, through March 30, 2009 and FCHV-adv vehicles manufactured December 12, 2008, through September 13, 2011. During manufacturing, a scratch may have occurred inside of the electrically driven water pump at the coil wire. The coil wire may corrode at the scratched portion.
Risk: The corroded coil wire may break and the water pump could stop. The corroded coil wire could cause a short circuit between coil wires and a possible open fuse, creating a stall-like condition of the hybrid system while the vehicle is being driven. This may increase the risk of a vehicle crash.
Fix: Toyota will notify its owners, and dealers will replace the electric water pump, free of charge. The recall began on January 17, 2013. Owners may contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331.
NHTSA Campaign: 09V388000
Component: VEHICLE SPEED CONTROL:ACCELERATOR PEDAL
TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.
Risk: A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.
Fix: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.

Owner-Reported Issues (20 of 2003)
Service Brakes →6 issues
Your brake actuator has developed an internal leak, which Toyota's diagnostic confirmed. You first noticed this in 2023 when the car wouldn't accelerate after braking during cruise control, followed by an intermittent red brake warning light. In 2024, both the brake and ABS warning lights came on steadily, and after sitting unused for months, a continuous loud beeping sound started when you restarted the car. The dealership estimates the repair at $3,205.40, which exceeds your vehicle's current market value of $2,000–$4,000.

Your check engine light came on with diagnostic codes C1256 and C1300, indicating a brake actuator failure. The brake actuator is a critical component of your braking system, and if it fails completely, your brakes could stop working while driving, which is a serious safety hazard. This appears to be a known issue that may require replacement of the brake actuator assembly.

Your brake actuator assembly is failing, which controls how your brakes apply pressure to stop the car. Toyota has acknowledged this as a known issue and offered warranty repairs in the past, but has since discontinued the program and won't repair or help with the problem. This could affect your ability to brake properly.

Your car's brake system warning lights (ABS, parking brake, and vehicle stability control) came on while you were driving on the highway. This indicates your brake actuator—the component that controls your braking system—likely needs to be replaced. The repair will involve replacing the brake actuator assembly, which is a significant component of your braking system.

Your Prius has a known problem with the ABS brake actuator that affects how the anti-lock brakes work. Your mechanic quoted $3,000 to repair it. Toyota has been quietly replacing these actuators under an unadvertised extended warranty program, but yours is outside that coverage window. The fix will likely involve replacing the ABS brake actuator assembly.

Your 2007 Prius's ABS brake actuator system is failing and needs replacement, which Toyota quoted at $5,000 at only 133,000 miles. You've noticed warning lights on your dashboard (red triangle, check engine, VSC, and exclamation mark) and a noise when braking, which a Toyota dealer confirmed is the brake actuator. Toyota says they sent you a recall letter in 2017 for free replacement under an extended warranty that covers cars up to 150,000 miles, but you never received it, and your dealer didn't inform you of the issue at that time. The fix will require replacing the entire ABS brake actuator assembly, which is a complex brake system component.

Your Prius is making frequent whirring noises from the front of the vehicle, especially when you press the brake pedal, occurring roughly every 10-15 seconds. The dealership diagnosed this as a brake actuator problem, which is a critical component in your hybrid's braking system. Repairs or replacement could cost $2,000-$3,000, and many regular mechanics won't work on hybrid brake systems. Fixing this will likely require specialized hybrid brake service or actuator replacement at a dealership.

Fuel/propulsion System →1 issue
Your Prius won't accept a full tank of fuel—it stops taking gas and overflows, causing fuel to spray forcefully out of the filler neck. The fuel gauge doesn't work, so you can't tell how much gas is in the tank, and in winter the car only accepts about 2 gallons before overflowing. This creates a fire hazard since fuel can spill near the filler area and potentially ignite. The issue appears to be a faulty fuel tank bladder (an internal component that controls fuel flow) that's preventing the tank from filling normally and causing fuel to back up and spray out.

Seat Belts →1 issue
Your driver's seat belt buckle has ejected a spring and plastic component, and the seat belt no longer fastens properly. This is a safety concern since the seat belt won't keep you secured in the event of a collision. The seat belt buckle assembly will likely need to be replaced.

Exterior Lighting,air Bags,seat Belts →1 issue
Your headlights spontaneously turned off while driving at night on an unlit interstate, causing you to lose control. Your car went onto gravel, flipped onto the median, and was totaled. The airbag did not deploy and the shoulder seat belt did not provide protection, resulting in your head hitting the steering wheel and causing skull and cervical fractures. Investigation of the lighting system and airbag/seat belt deployment mechanisms will be needed.

Exterior Lighting,air Bags →1 issue
Both of your headlights failed while driving at night on a dark highway, forcing you to slow down from 60 mph to find a safe place to pull over. An accident occurred during this process, and neither airbag deployed when the crash happened, leaving you injured. Fixing this issue would involve replacing the failed headlight assemblies and having the airbag system inspected.

Electrical System, unknown Or Other →1 issue
Your car's 12-volt battery can fail completely while you're driving with no warning light to alert you beforehand. When this happens, your car suddenly loses power steering, braking becomes difficult, acceleration stops, and the vehicle becomes disabled on the road—creating a dangerous situation with no advance warning. Instead of a clear battery warning, confusing dashboard lights appear (including brake warning lights and a transmission error message) that don't indicate the actual problem. To prevent this dangerous situation, Toyota should add a dedicated low-battery warning light that alerts you before the battery dies.

Tires →1 issue
Your tire developed a puncture during highway driving, and when you took it in for repair, the technician discovered the steel belt inside the tire was visible and protruding through the rubber. The repair facility recommended immediate replacement due to the risk of a blowout, but didn't have a matching tire available. You've contacted the tire manufacturer twice since September 2024 without receiving a response. The tire replacement will involve purchasing a new tire that matches your vehicle's original 185/65R15 size specification.

Exterior Lighting →1 issue
You're experiencing severe glare from oncoming LED headlights that are at eye level with your Prius, making it difficult to see the road ahead. The bright lights coming directly toward you are creating a significant safety concern that impairs your ability to drive safely. Addressing this may involve adjusting your headlight aim, installing an anti-glare visor, or using polarized sunglasses while driving.

Power Train,engine →1 issue
Your car keeps triggering a P1116 error code even after you've had it serviced, and you've experienced loss of power while driving uphill where the engine wouldn't accelerate properly—though the car would start and shift into gear. This suggests a persistent engine control issue that needs diagnostic investigation to identify the root cause. The repair will likely involve diagnosing the engine control system, checking sensors, and possibly replacing components related to fuel delivery or ignition timing.

Electrical System →1 issue
Your dashboard display sometimes doesn't turn on when you start the car, which means you can't see your speedometer, warning lights, fuel level, or mileage. This same electrical problem can also lock your trunk so it won't open, and occasionally prevent you from shutting off the engine. The issue has been happening intermittently for over a year and is getting worse, though there are no warning signs before it occurs. The problem likely involves repairing or replacing the combination meter assembly and its electrical connections.

Structure →1 issue
Your catalytic converter is easily accessible and vulnerable to theft because there's no protective guard underneath your car. You may not realize it's been stolen until you drive and notice your engine burning and smell smoke from the theft damage. The owner is requesting a recall to add anti-theft protection to all vehicles and prevent further thefts. Fixing this would likely involve installing a protective shield or cage around the catalytic converter.

Electrical System, engine, hybrid Propulsion System →1 issue
Your 2007 Prius won't go faster than 25 MPH and is showing check engine and hybrid battery warning lights. The dealer found that your hybrid battery was severely drained and retrieved error codes P3000 and P0A80 related to the battery pack. Even after replacing the battery at an independent shop, the problem continued. You believe the issue started after an oil change where your car was overfilled with oil. The fix will likely involve diagnosing whether the battery failure is truly the root cause or if the overfilled oil damaged engine sensors or the hybrid system's electronics.
