My 2018 Toyota Tacoma started showing a "forward collision detection system malfunction" "Contact your dealer" light came on that also indicates system is not on/functioning. I went to the dealer and was told the Millimeter Wave Radar Sensor Assembly #88210-04020 wad defective. My replacement cost and labor would be $1,543. I refused the service as I wanted to better understand the issue. I found that the exact item had fallen under a Toyota KOB recall. The same dealer had performed the recall in 2022. I believe the dealer should warranty the recalled part and service. It also appears the recall was performed incorrectly where a backing foam / seal that was to protect the sensor from moisture was installed incorrectly according to the recall recommendations.

While driving on 3 lane interstate, with the adaptive cruise control on, while passing a semi truck the front collision brake system activated. I was doing 72 miles an hour when it happened. The result was almost got rear-ended by another vehicle when my truck applied the brakes unexpectedly. The semi did not cross into my lane. My lane was clear. 0

2018 Toyota Tacoma problem. I purchased the truck in October of 2018. On July 31, 2021 @ 3:50 pm While driving home I was in cruise control @ 82 mph when the display showed malfunction. The truck went from 82 mph to 60 within seconds again. A car behind me was able to change lanes to avoid an accident. The trucks display readings were Malfunction of headlight, radar cruise control unavailable & Lane departure alert malfunction and visit the dealer. After getting home I shut the truck off and restarted it. The malfunctions had cleared again. I made an appointment with the dealership for August 5, 2021.They worked with Toyota representatives to trying to solve the problem but couldn’t identify it. The conclusion was when it happens again bring it to the dealership (if possible) before shutting it off so they can do a diagnostic test while the truck is still running and showing the malfunction display. Luckily, when it occurred I was on a road with every little traffic. My problem now is that I don’t trust driving it using cruise control in normal traffic or at night not knowing if the headlights will shut off or the cruise control will shut down. This is the second time this has occur. The first time was in Aug 2019. I took to the dealer but they couldn’t identify the problem. My thought at the time was it may have been a one time event but it was not. I have pictures of the truck’s displaying the malfunctions on both events.
