Your accelerator pedal has gotten stuck at full throttle three separate times because it's sliding behind the floor mat, even though the mat appears to be in its correct position. This causes your car to accelerate uncontrollably until you manually free the pedal from under the mat. You may need to replace or modify the floor mat, or have the pedal assembly checked to prevent it from moving behind the mat.
