On May 18, 2025, 4:00pm, my wife was applying brakes to stop for a stop sign at a T-intersection. Suddenly, the car lurched forward and accelerated uncontrollably. Despite her attempts to stop, her vehicle crossed four traffic lanes, vaulted over curb, nosedived into turf, and then crashed through brick wall into a fire station.

Your SRS airbag warning light is illuminated on the dashboard. A Toyota dealer confirmed that a faulty radio head unit is preventing the SRS airbag system from being recognized. If you're in a collision, your airbags may not deploy, leaving passengers unprotected. The fix will likely involve replacing the radio head unit to restore proper airbag system communication.

Your passenger-side airbag warning light turns on and off randomly, even when nothing is on the seat. The dealership says they can only fix it if the warning light is actively showing when you bring the car in for service.

After starting the van, an error pops up: "SRS Airbag System Malfunction Visit Your Dealer." This occurs a day after turning the car on and off and driving for several hours. The next day, it may come back on, or it may not. I have taken it to a dealership, and they stated it was a firmware update they completed.
