NHTSA Campaign: 20V033000

Component: AIR BAGS:FRONTAL:DRIVER SIDE:INFLATOR MODULE

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 1998-2000 RAV4, 1998-1999 RAV4 EV and Celica and 1997-1998 Supra vehicles. These vehicles were equipped with Non-Azide Driver air bag Inflators (NADI) and do not contain phase stabilized ammonium nitrate (PSAN) propellant. Due to a manufacturing issue, the NADI inflators may absorb moisture, causing the inflators to rupture or the air bag cushion to underinflate.

Risk: In the event of a crash necessitating air bag deployment, an inflator rupture may result in metal fragments striking the driver or other occupants. An underinflated air bag may not properly protect the occupant, and increases the risk of serious injury or death.

Fix: Toyota will notify owners, and owners will have the option to self-inspect, or have a dealer inspect the steering wheel on their vehicle to determine if their vehicle may be equipped with an affected inflator. Potentially affected steering wheel serial numbers will be inspected by a dealer. Based on the inspection, dealers will replace the front driver air bag inflator with an alternate one, as necessary. These repairs will be performed free of charge. The recall began August 28, 2020. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's numbers for this recall is 20TB01 and 20TA01.

NHTSA Campaign: 00V212000

Component: SERVICE BRAKES, AIR:DISC:ROTOR

VEHICLE DESCRIPTION: SPORT UTILITY ELECTRIC VEHICLES. THE FRONT DISC BRAKE ROTOR, WHEN EQUIPPED WITH STEEL WHEELS, CAN DEVELOP STRESS FRACTURES IF THE LUG NUTS ARE TIGHTENED WITH EXCESSIVE TORQUE.

Risk: THE DISC ROTOR COULD BREAK (WHERE IT MOUNTS TO THE WHEEL), RESULTING IN A LOSS OF BRAKING ACTION ON THAT WHEEL, INCREASING THE RISK OF A CRASH.

Fix: DEALERS WILL REPLACE THE DISC BRAKE ROTORS. IF THE VEHICLE IS EQUIPPED WITH STEEL WHEELS, TOYOTA WILL REPLACE THE FRONT DISC BRAKE ROTORS AND STEEL WHEELS WITH NEW DISC BRAKE ROTORS AND NEW ALUMINUM WHEELS. IN ADDITION A CAUTION LABEL THAT READS, "USE NEW GENUINE TOYOTA ALUMINUM WHEELS." WILL BE AFFIXED. VEHICLES ALREADY EQUIPPED WITH ALUMINUM WHEELS WILL ONLY REQUIRE CAUTION LABEL INSTALLATION.

Your transmission is making strange noises and slipping without warning, which first happened while driving on a highway. After this incident, the transmission continues to make unusual sounds and slip when you try to drive. The car is currently parked because you're unsure what's causing the problem. A transmission repair or rebuild will likely be needed to address the slipping and noise.

Your RAV4 has experienced jerking and power loss while driving on the highway on multiple occasions, forcing you to pull off the road. The Toyota dealership said they couldn't diagnose the problem because the check engine light isn't on. This intermittent loss of power could put you and other drivers at risk. The fix will likely involve a thorough diagnostic scan of the engine and fuel system, even without a check engine light present.

Your 1998 RAV4's fifth gear keeps popping out of engagement while driving on the highway, even when you try to hold the shift stick in place. This forces you to keep your hand on the shifter to maintain fifth gear, which diverts your attention from driving and can cause unexpected jerking that could be hazardous to you and others on the road. You had the fifth gear replaced at a transmission specialist at your own expense after Toyota did not recall this known issue. Fixing this problem typically involves replacing the fifth gear assembly or internal transmission components.

Your RAV4 has started having acceleration problems since reaching 120,000 miles. The issue happens about a dozen times, mostly when you're stopped at a traffic light or trying to pull into traffic, though it may have occurred once or twice while driving. The problem likely involves the fuel delivery or ignition system and may require diagnostic testing to pinpoint the exact cause.

Your RAV4 was hit on the front passenger side during a crash, but the airbags didn't deploy when they should have. Two passengers were injured — one with a black eye, leg bruises, and head lacerations requiring stitches, and another with two broken ribs. The vehicle was totaled and not repaired, so the airbag system was never diagnosed.

While your wife was slowly approaching a parking spot at about 5-10 mph, your RAV4 suddenly accelerated on its own, causing her to jump a curb and crash into a store column. The airbags deployed, and the impact was severe enough to total the car and injure your wife with chest bruises that required emergency room treatment. The sudden acceleration issue may involve the throttle control system or engine management components.

Your gas pedal assembly broke away from the firewall (the metal barrier between the engine and cabin), though the bolts remained attached to the pedal itself. This would prevent you from being able to accelerate the vehicle. The fix will likely involve reattaching or replacing the gas pedal mounting bracket and firewall attachment points.

Your accelerator pedal isn't responding normally when you press it—the engine won't engage until you press hard, causing the car to jerk forward suddenly. You might also notice the vehicle responds better when you pump the pedal up and down rather than pressing it smoothly. A service center fixed this before by lubricating the pedal, though the problem has recurred over time.

You hear a knocking noise in your steering wheel when making slow turns, like at intersections. This problem has been getting worse since you bought the car in 2008, and a Toyota dealer says it's a safety issue where the steering could fail without warning. Toyota has technical service bulletins on how to fix it, but they won't cover the $2,800 repair cost under warranty.

CrashFiled: Jun 2012

During a multi-vehicle crash at highway speed, your airbags did not deploy, and your seat belts failed to lock on impact, resulting in injuries to all three passengers. The vehicle was destroyed in the collision and was not inspected to determine why the safety systems malfunctioned. This issue involves both the airbag system and seat belt locking mechanisms.

Filed: Apr 2011

Your brakes failed to work when you tried to stop at a red light, and the vehicle lurched forward instead. Pumping the brake pedal a second time didn't help—the car actually accelerated. Only on the third firm brake application did the vehicle finally stop, but you nearly hit the car ahead of you. This brake failure may involve the hydraulic system and could require inspection of the master cylinder, brake lines, or brake fluid.

Even after you replaced the brake pads during maintenance, your brakes still feel loose or unresponsive at times, especially after driving on the highway. This could indicate a problem with the brake system beyond just the pads, such as air in the brake lines, worn rotors, or issues with the brake calipers. You should have a mechanic inspect the entire brake system to identify and fix the underlying cause.

Your engine is burning through oil much faster than normal — you're losing more than a quart every 1,200 miles. You first reported this to the Toyota dealer at 149,000 miles, but they wouldn't investigate it. Now at 200,000 miles, the dealer says you need to pay over $3,000 to fix it and has admitted the problem is likely worn piston rings or pistons. This appears to be a manufacturing defect that Toyota knew about but isn't covering under warranty.
