VEHICLE SPEED CONTROL:ACCELERATOR PEDAL

TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.

Fix: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.

While turning into a parking lot at low speed with your foot on the brake pedal, your Prius suddenly revved to high RPM and accelerated forward aggressively, causing you to lose steering control. Even with hard braking pressure, the brakes did not respond and the vehicle hit an electrical gate control box. The vehicle had all original equipment with no aftermarket modifications or floor mats.

The inverter water pump failed while I was driving 50mph, it caused the car to stop working and the brakes malfunctioned as well. When I tried to hit the brakes I had to hit them a lot harder and it still acted like it wasn’t going to stop. I was lucky enough that the light up ahead was still green and the cars in front of me started going forward.

MY ANKLE SUFFERED A SEVERE TWIST FROM TRYING TO APPLY BRAKES. THERE WAS PLENTY OF ROOM TO STOP BUT THE BRAKES WOULD NOT REACT TO SLOW DOWN VEHICLE. THERE WAS A CAR COMING IN THE ONCOMING LANE SO I COULD NOT GO AROUND VEHICLE IN FRONT OF ME. MY VEHICLE WAS IN MOTION IN THE CITY AND I WAS GOING STRAIGHT. I TRIED TO APPLY THE BRAKES BUT THE CAR DID NOT STOP, I APPLIED THE BRAKE EVEN HARDER, STILL THE CAR DID NOT STOP BUT CONTINUOUS TO MOVE FORWARD. IT WAS TRAFFIC TIME ON A MONDAY MORNING, SO I WAS GOING ON 15MPH. WHEN I FOUND THAT MY BRAKE DID NOT RESPOND, I IMMEDIATELY MOVED TO THE CURB AND CALLED AAA. DEALER DID CHECK MY VEHICLE AND SAID THERE IS BREAK ACTUATOR ISSUE AND NEEDS TO BE REPLACE WITH A PRICE TAG OF 3823 USD. SPOKE TO TOYOTA CUSTOMER SERVICE, AND THE ASSOCIATE TOLD THAT THIS IS A WEAR AND TEAR ISSUE. HOW COME THE BRAKE ACTUATOR BECOME FAULTY ON A 6 YEAR OLD CAR, WHICH WAS ADVERTISED A SAFE CAR. THEN THE BRAKES STARTED WORKING AGAIN AND I COULD STOP THE CAR. THIS HAS HAPPENED 3 ADDITIONAL TIMES SINCE THEN IN DIFFERENT PLACES DRIVING AROUND TOWN, PROBABLY AFTER HITTING SLIGHT BUMPS OR A FEW PIECES OF GRAVEL. ONE OF THOSE HAPPENED AS I WAS GOING TO THE TOYOTA DEALER TO HAVE IT CHECKED OUT. THE TOYOTA SERVICEMAN COULD NOT FIND ANY CODES THAT INDICATED THAT THERE WAS A PROBLEM. ON THIS DATE WHILE DRIVING ON THE INTERSTATE 5 NEAR LOS ANGELES MY PRIUS MADE A LOUD NOISE AND STARTED SHUTTING DOWN AS I WAS DRIVING ON THE FAR LEFT LANE. THE CAR IMMEDIATELY SLOWED DOWN AND THE VSC ICON AS WELL AS THE CHECK ENGINE LIGHT ICON BECAME VISIBLE, HOWEVER THE ODOMETER WAS BLANK AND I COULD NOT TELL WHAT SPEED I WAS TRAVELING, THE CAR WAS COMING TO A COMPLETE STOP. ONCE I MANAGED TO GET TO THE FAR RIGHT SHOULDER THE CAR MADE A STOP. I WAS ALMOST REAR ENDED BY A BIG RIG WHICH HAD TO SWERVE TO THE LEFT SIDE IN ORDER TO AVOID HITTING MY CAR. IT WASN'T UNTIL THEN THAT I NOTICED THE RED ENGINE LIGHT ICON DISPLAYED ON THE CONTROL PANEL NOT THE ODOMETER. I HAVE NEVER SEEN THIS WARNING LIGHT BUT AFTER LOOKING AT THE OWNER'S MANUAL IT CLEARLY STATED THAT ONCE THIS LIGHT APPEARS YOU NEED TO GET THE VEHICLE OFF THE ROAD AS SOON AS POSSIBLE OR IT WILL CAUSE AN ACCIDENT. THIS WAS THE FIRST TIME I NOTICED THIS SIGN AND IT APPEARED AFTER THE FACT. THE CAR HAD NO ENGINE OIL, WHICH WAS RIDICULOUS SINCE I HAD NOT NOTICED ANY LEAKS AND I TAKE MY CAR IN EVERY 2 MONTHS TO THE DEALER FOR AN OIL CHANGE. THE DEALER HAD NO EXPLANATION OTHER THAN IT DID NOT HAVE ANY LEAKS AND THEY WERE NOT ABLE TO VERIFY WHEN THE RED ENGINE OIL CAME ON. THIS HAPPENED ON 5/30/14, MY LAST VISIT TO THE DEALER WAS ON 4/12/14 AND AT THE TIME THEY DID NOT DETECT ANY PROBLEMS WITH THE OIL LEAKING. THIS CLEARLY MAKES NO SENSE AND THEY ARE REFUSING TO TAKE RESPONSIBILITY. TOYOTA OWNERS SHOULD BE VERY CAUTIOUS AS THIS SHOULD HAVE NEVER HAPPENED THE SENSOR SHOULD HAVE GIVEN ME A WARNING PRIOR TO THE CAR SHUTTING DOWN. IT ACTUALLY HAPPENED TWO TIMES WITHIN A MONTH. IN ONE CASE, I WAS TURNING LEFT TO TAKE A RAMP WITH A VERY LOW SPEED AROUND 5 MPH. EVEN THOUGH SOME PARTS OF THE ROAD WAS ICY, BUT THE CAR DIDN'T SLIP. IT WAS EXACTLY A SEVERAL SUDDEN PUSHES, UNINTENDED ACCELERATION, WITH MY FOOT ON THE BRAKE ONLY. I WAS SO SCARED BECAUSE I COULDN'T CONTROL THE CAR FOR FEW SECONDS, AND I THOUGHT I TOTALLY LOST THE CONTROL OF THE CAR. I TRIED TO STEER THE CAR SAFELY AND TURN ON MY EMERGENCY LIGHT. CHECKED THE BRAKE AGAIN, IT WAS WORKING AFTER THAT. THE SECOND TIME, HAPPENED TO ME WHEN I WAS COMING OUT OF SCHOOL PARKING AND DIDN'T HAVE ANY SPEED AT ALL. AGAIN AROUND 5 MPH. SCHOOL IS LOCATED IN RESIDENTIAL AREA. WHEN YOU GET OUT OF THE PARKING, YOU CAN ONLY TURN RIGHT, THEN THERE IS AN INTERSECTION WITH A STOP SIGN ALMOST 50 FEET AWAY. ON THE DAY OF INCIDENT, I TRIED TO STOP BEHIND THE STOP SIGN WITH NO SPEED AT ALL, BUT SUDDENLY THE CAR GOT THE SAME CONTINUOUS AND SUDDEN PUSHES. IT WAS ACCELERATED UNINTENDED. MY FOOT WAS COMPLETELY ON THE BRAKE, BUT I LOST THE CONTROL AGAIN, AND I WAS SO LUCKY THAT DAY THAT THERE WERE NO CARS AROUND THE INTERSECTION, I TRIED TO STEER THE CAR TO THE LEFT, TURNED ON MY EMERGENCY LIGHT, AND TRIED TO PUSH THE BRAKE AGAIN AND AGAIN. AFTER FEW SECONDS, I FOUND IT WORKING. I CALLED THE TOYOTA THE SAME DAY, AND THEY ARE IN THE PROCESS OF SCHEDULING ME FOR INSPECTION. AGAIN, EVEN THOUGH THE SURFACE WAS SLICK WITH ICE, BUT I DIDN'T HAVE SPEED, AND IT DIDN'T LOOK LIKE ABS IS WORKING. I EXACTLY KNOW HOW ABS WORKS, SPECIALLY WHEN IT IS SNOWY OR ICY. IDLE TO HIGH REV AND IDLE AGAIN FOR A FEW TIMES AND THAT HAPPENED FOR ABOUT 2 WEEKS WHILE ON THE ROAD. THE VEHICLE STARTED TO ACCELERATE BY ITSELF AT THE FIRST WEEK OF NOV 2013 AND HAPPEN AGAIN ON HIGHWAY 101 SOUTH IN MILL VALLEY CA AROUND 8 PM. I HAVE CALLED CITY TOYOTA AND EXPLAINED ABOUT IT BUT THE SERVICE DEPARTMENT JUST TOLD ME TO GET APPOINTMENT TO CHECK IT UP. AS I ASSUME MYSELF AS WELL EXPERIENCED DRIVER WITH 25 YEARS OF DRIVING MULTI TYPE OF VEHICLES, IT IS NOT DRIVER ERROR NOR THE PEDAL NOR THE FLOOR MATS. IT IS MECHANICAL OR ELECTRICAL PROBLEM. *TR

ON TWO SEPARATE OCCASIONS MY 2008 TOYOTA PRIUS HAS DONE THE AUTOMATIC ACCELERATION PROBLEM. THESE WERE BOTH LONG BEFORE RECENT REPORTS IN APRIL OF LAST YEAR. THE FIRST TIME WAS ON ROUTE 84 BETWEEN THE WHITE PLAINS AND CARMEL EXITS. MY VEHICLE ACCELERATED TO BETWEEN 80 -85 MPH. I WAS IN SOME TRAFFIC AND FLASHED MY LIGHTS AT OTHER VEHICLES IN THE PASSING LANE BECAUSE I COULD NOT STOP! WHEN I REGAINED CONTROL OF MY VEHICLE SEVERAL MINUTES LATER, I CALLED MY DEALERSHIP AND THEY HAD ME BRING THE CAR RIGHT IN AND THEN TOLD ME NOTHING WAS WRONG WITH IT. A DIAGNOSTIC TEST WAS PERFORMED AND THE DEALER STATED THAT THE COMPUTER CODE INDICATED THAT THE DRIVER WAS "DEPRESSING THE ACCELERATOR AND BRAKE PEDALS SIMULTANEOUSLY." THE CONTACT VERIFIED THAT NEITHER HE NOR THE OTHER DRIVER OF THE VEHICLE HAD EVER OPERATED THE VEHICLE IN SUCH A MANNER. THERE WERE NO ADDITIONAL REPAIRS MADE TO THE VEHICLE. THE FIRST TIME THEY SAID THEY DID NOT KNOW WHAT IT WAS SINCE THEY COULD NOT REPRODUCE IT. THE SECOND TIME THEY SAID IT MUST HAVE BEEN BECAUSE OF THE FLOOR MATS. TOYOTA PRIUS PROBLEM - CRUISE CONTROL MALFUNCTION: ON 2 JAN 2010 (ABOUT 0600 HRS) I WAS DRIVING A RENTED PRIUS (ALAMO) FROM PALM BEACH, FL TO MIAMI, FL. I WAS USING CRUISE CONTROL AND GOING THE LISTED SPEED LIMIT FOR I-95 (65MPH). THE CAR SEEMED TO BE PERFORMING CORRECTLY FOR THE FIRST 35 PLUS MILES BUT THEN I GOT INTO HEAVY TRAFFIC AND HAD TO BRAKE. THE CRUISE CONTROL RELEASED NORMALLY AND I SLOWED TO ABOUT 60 MPH. WHEN THE TRAFFIC CLEARED I PUSHED THE CRUISE CONTROL "RESUME" BUTTON AND THE PRIUS STARTED TO ACCELERATE AS NORMAL. HOWEVER, THE ACCELERATION DID NOT STOP AT THE ORIGINAL SPEED I HAD SET AND HAD BEEN DRIVING AT (65 MPH) BUT CONTINUED TO CLIMB TO 70 MPH AND HIGHER TO NEAR 75 MPH WHEN I HAD TO HIT THE BRAKES AND SHUT THE CRUISE CONTROL OFF TO SLOW THE CAR. AFTER THAT I DID NOT USE THE CRUISE CONTROL FOR THE REMAINDER OF MY TRIP. WE WERE STOPPED AT A NEIGHBORHOOD INTERSECTION. AS WE BEGAN TO LET OFF OF THE BRAKE TO MAKE A LEFT HAND TURN, THE CAR ACCELERATED UNCONTROLLABLY. DEPRESSING THE BRAKE HAD NO EFFECT ON THE ACCELERATION. BEFORE WE TURNED THE WHEEL TO THE LEFT TO BEGIN THE TURN, THE CAR HAD SPED ACROSS THE 35 FOOT STREET, JUMPED A 6 1/2 INCH CURB, WENT THROUGH A FENCE, ACROSS ANOTHER STREET, AND WAS STOPPED WHEN IT HIT A SECOND FENCE. THE TOTAL DISTANCE TRAVELED FROM BEING STOPPED AT THE CORNER UNTIL THE VEHICLE WAS STOPPED BY THE SECOND FENCE WAS 81 FEET. THE STREETS WERE DRY AND THERE WERE NO SKID MARKS ON THE ROAD. HOWEVER, DUE TO PREVIOUS RAINS, THE GRASS AREAS WERE MOIST. THERE ARE NO TIRE MARKS IN THE GRASS FROM THE CURB UNTIL APPROXIMATELY 6 FEET BEFORE THE STOPPING POINT. THE FRONT LIFT OF THE PRIUS IS AROUND 7 1/2 INCHES. THE VEHICLE HAD TO HAVE SIGNIFICANT MOMENTUM TO GO OVER A 6 1/2 INCH CURB IN JUST 35 FEET. WE FILED A CLAIM WITH OUR INSURANCE ON JAN. 7 AND REQUESTED THAT THE VEHICLE BE INSPECTED FOR ACCELERATOR ISSUES. THE CAR WAS TOWED TO TOYOTA WHOM, UPON THEIR OWN INVESTIGATION, STATED THAT THERE WAS NO EVIDENCE OF ACCELERATION MALFUNCTION. THE CAR IS BEING REPAIRED AT A BODY SHOP WITH INITIAL DAMAGE ESTIMATED AT OVER $6000. WE HAVE PHOTOGRAPHS OF THE DAMAGE TO THE CAR AS WELL AS THE SCENE OF THE ACCIDENT AND MEASUREMENTS OF DISTANCES TRAVELED. IN 2009, WE RECEIVED A RECALL LETTER FROM TOYOTA CONCERNING FLOOR MAT ISSUES WITH OUR PRIUS. WE INSPECTED OUR FACTORY-INSTALLED FLOOR MATS, AS INSTRUCTED BY THE RECALL LETTER, AND WERE AWAITING THEIR 2ND NOTIFICATION WHEN THE "CAMPAIGN REMEDY" BECAME AVAILABLE. THE FLOOR MAT ON THE DRIVER'S SIDE HAS A LOCKING MECHANISM AND COULD HAVE, IN NO WAY, OBSTRUCTED THE ACCELERATOR PEDAL. THE CAR SHOULD BE REPAIRED AND READY NEXT WEEK. A POLICE REPORT WAS FILED AND THERE WERE NO INJURIES. HE DOES NOT WANT TO DRIVE THE VEHICLE AGAIN AND FEELS THAT THE ISSUE SHOULD BE TAKEN SERIOUSLY BY THE MANUFACTURER. ON A SLIGHT INCLINE, WITH FRONT WHEELS TURNED, CAR STARTED NORMALLY THEN ACCELERATED BY ITSELF AND WAS DIFFICULT TO STOP WITH BOTH FEET ON BRAKE PEDAL. ACCELERATION WAS NOT SUDDENLY FAST BUT WAS INCREASING WHEN I TURNED CAR INTO GRAVEL DRIVE LEADING UP A HILL. CAR STOPPED WITH BRAKING AND WAS SUCCESSFULLY TURNED OFF IN THE MANNER REQUIRED BY MANUAL. TL*THE CONTACT OWNS A 2008 TOYOTA PRIUS. WHILE DRIVING 75 MPH WITH THE CRUISE CONTROL ACTIVATED, THE CONTACT ACCELERATED PAST 80 MPH TO PASS ANOTHER VEHICLE. THE VEHICLE WOULD NOT SLOW DOWN AFTERWARDS AND ACCELERATED PAST 90 MPH. SHE DEPRESSED THE BRAKE PEDAL, BUT THE CRUISE CONTROL WOULD NOT DISENGAGE. THE CONTACT ATTEMPTED TO TAP THE BRAKE PEDAL AND SHUT OFF THE CRUISE CONTROL MANUALLY, BUT WAS UNSUCCESSFUL. SHE MANAGED TO PULL TO THE SHOULDER OF THE ROAD AND SHUT OFF THE ENGINE. UPON RESTARTING, THE VEHICLE ACCELERATED AGAIN WITHOUT DEPRESSING THE ACCELERATOR PEDAL. SHE ATTEMPTED TO RESET THE CRUISE CONTROL TO 60 MPH, BUT IT WOULD NOT RESPOND. THE CONTACT WAITED 30 MINUTES BEFORE RESTARTING THE VEHICLE AND IT APPEARED TO HAVE MOVED FROM THE LOCKED POSITION OF ACCELERATION. THE FAILURE AND CURRENT MILEAGES WERE LESS THAN 7,250.

I HAVE A 2008 TOYOTA PRIUS WITH 10K MILES ON IT. SEVERAL WEEKS AGO WHILE DRIVING 65 ON THE HIGHWAY, THE CRUISE CONTROL KICKED OUT AND THE CAR RAPIDLY DECELERATED, ALMOST LIKE IT WAS BRAKING. I COULD NOT RE-SET THE CRUISE CONTROL AGAIN UNTIL AFTER STOPPING AND RESTARTING THE CAR. THE PROBLEM HAS AGAIN HAPPENED 3 MORE TIMES IN THE PAST WEEK.
